Couples planning to participate in an IVF cycle at the Fertility Center are required to sign a number of notarized consents. These legal forms document agreement and grant permission for procedures, actions and decisions. Click below to become familiar with the standard consent forms:

It’s very important for couples considering assisted reproduction to begin to think about and discuss the complex issues involved. The Fertility Center recommends reviewing options together with your spouse to reach agreement before signing any forms. You are welcome to use our staff notary free of charge to help ensure your privacy and confidentiality. As a patient, you’ll have several opportunities to talk with both doctors and the embryologist, who will answer questions and address concerns about consents. All consents must be signed, notarized, copied and filed in charts before a couple can proceed with an IVF cycle. Some patients prefer to pick up consent forms at the Fertility Center to sign and have notarized outside our office at their convenience.
Patients who participate in a cycle using "frozen" embryos are required to sign one additional consent (separate from the original five IVF consents) before or on the day of the lining check appointment prior to transfer.
