New Patient Forms

The Fertility Center utilizes the eIVF patient portal, an electronic medical records system. After you book your first appointment, you will receive email instructions for accessing the portal and completing the required information. During treatment, you will also be able to access appointment schedules, cycle instructions and lab results.

If you are unable to complete the online registration before your appointment, please arrive at least 30 minutes early to complete it in our office.

When you arrive at our office, you will be asked to sign in and complete additional paperwork that requires signatures. These forms are provided below so you can complete them ahead of time if you prefer.

    Sorry, Fertility Center does not have a sperm donor program. We’re not aware of any local sperm banks in the area. The main sperm banks we work with are Seattle Sperm Bank and Fairfax Cryobank. They may be able to give you more information. Have a great day!
